Making the conditions necessary for mental health accessible to everyone.

Since 2003, form communities has been at the forefront of transformative community-based mental health support. So much of what we need to thrive comes from being part of a supportive community, and form communities aims to make this possible for everyone. Sometimes form directly provides services. At other times, the focus is simply on elevating great organizations or removing community and system barriers.

Three core areas of work

We focus on creating supportive spaces, fostering a skilled workforce, and scaling up impactful services to meet the needs of our communities.

Direct mental health supports

Connecting individuals to supportive communities, recovery opportunities, and support to meet life goals, such as employment or housing changes.

Behavioral health workforce development

Helping passionate people transform their lived recovery experiences into impactful, community-focused behavioral health careers.

Impact scaling and sustainability services

The needs in our communities are too great for us to compete with each other. Form communities strives to share every lesson we’ve learned with other organizations so they can have similar impacts for those they serve.

Why Houston Peer Success?

Fun fact: At 7.1M people, if the Houston metro area was its own state, it would be the 15th biggest state in the USA–just smaller than Tennessee, and a bit bigger than Massachusetts.

Houston is large, and so is the need for accessible recovery resources.

Through our PeerForce and Peer Academy programs, we found a tremendously collaborative spirit in many agencies in Houston–many of whom expressed common needs and goals. Even though form communities operates direct service programs in San Antonio and the Rio Grande Valley, we thought that the best way to support the Houston community was to help bring together the great agencies already doing impactful work–not necessarily creating another direct service program. 

Thus, Houston Peer Success was born.

The goal is to leverage our experience, access, and privilege to help other like-minded agencies succeed!
